In order to evaluate this year and proceed with the 2^nd year disbursement we are sharing the following documents:

 

Year 1 Evaluation:

1.      Expenditure report Y1

2.     CCM Funding Performance Framework

3.     Audit report

4.     Bank statements and reconciliation both in USD and GEL

 

Year 2 Documentation agreed with all CCM members:

5.     Costed workplan

6.     CCM Funding Performance Framework Y2 with suggested changes

 

On the same note please be kindly informed that according to the bank statement the closing balance from Y1 and from the previous year is in the amount of 27,680.71 USD and 1,072.51 GEL.

With this letter we are addressing you with the request to allow us to proceed with spending of left amount mentioned above before disbursement of Y2 funds, in order to ensure that the CCM activities planned including salaries in Y2 of the current funding agreement are not delayed.

Thank you for your message.

Thank you for having shared again the Specimen Signature: I confirm that no additional specimen is now required.

For what concerns the endorsement of Year2 budget, please note that normally it is not required: we ask for the endorsement only before the approval of Year1 budget. The endorsement of Year2 budget is necessary only if there are substantial changes in the fixed costs with respect to Year1.

Kindly ask you to confirm whether we have to submit  completed and scanned Specimen Signature document for the recipient of funds, taking into the consideration that we have already submitted signature specimen document together with the signed funding agreement on May 2017? Please see the evidence in the attached file.

Could you please also confirm if we have to submit CCM endorsement form from online platform (signed by all CCM members) for Y2 budget?

The first year of the CCM Georgia Funding Agreement GEO-CFUND-1708 is coming to an end on the 28^st February 2018. In order to evaluate this year and proceed with the 2^nd year disbursement we will request you the following:

 

-          Year 1 Evaluation. You can find attached the document “GEO-CFUND-1708_ExpenditureReport_Y1. Please fill the yellow and grey cells of the Tab “Expenditure_Y1”.

Additionally, in order to evaluate the CCM Funding Performance Framework, you can find attached the document “GEO-CFUND-1708_PerformanceFramework_Evaluation_Y1”, please fill the tab “Evaluation_Y1”.

 

-          Year 2 Documentation. You can find attached the document “GEO-CFUND-1708_CostedWorkPlan_Y2”. Please fill the year 2 on the tab “CostedWorkPlanYear2” including any important information/reference in comments.

-          Please also confirm the information in the document “GEO-CFUND-1708_Performance Framework_Y2” that refers to the indicators for Year 2.

-          Please complete and scan the attached Specimen Signature document for the recipient of funds.

-          GEO-CFUND-1607- Final report and outstanding Management measures under Agreement: Please confirm cash balance of USD 18, 319 as of the 28^th February 2017.

 

In order to disburse in the shortest delays, please provide us with the documents by March 9. We will be grateful if you avoid any changes in the format of the templates.
