Dear Sopo,

 

Attached you can find the electronic versions of the candidature forms for Foundation Prizes in 2019.

 

Procedure for proposing candidates:

Each national health administration may nominate only one candidate per Prize. Nominations must be accompanied by adequate justification and the most pertinent documentation as evidence of the work carried out by the candidate.

 

The candidature of a current or a former member of the WHO Secretariat or of a current member of the Executive Board of the World Health Organization will not be accepted.

Candidates put forward in past years, but not selected, may be nominated again for the awards in 2019.

Nominations for candidates for any of the above-mentioned Prizes, should be sent, together with the candidature form(s), completed either in English, French or Spanish and signed by the competent national health authority, in time to reach WHO headquarters in Geneva not later than 8 November 2018. It will not be possible to consider nominations received after this closing date.

More information on the awards may be accessed on the Internet through the WHO website (http://www.who.int/governance/awards). Requests for further information and for electronic forms can also be sent to foundationprizes@who.int.

Nominating candidates for the prizes is an option/opportunity and is not obligatory.
