Based on your last filled report and questionnaire, would you be so kind to provide us with several additional information/quarries:

 

1.      You indicated that Armenian side has received 2999.286 bottles of Sovaldi, according to signed agreements you received 3000 bottles, please explain the difference?

2.      According to the report that you have submitted patients were ongoing three-month treatment regime, which means that 2958 bottles of Sovaldi should have been used,  + 37 bottles of Sovaldi were issued to patients who had stopped the treatment, in total 2995 bottles of Sovaldi should have been spent. The report tells us that there are no drugs left in stock, when there have to be 5 Bottles left, please explain discrepancy.

Dear All,

 

To summarize the project outcomes, we would like to clarify several issues:

 

1.      In total how many Bottles of Sovaldi have you received?

2.      JUNE  - There are two different numbers indicated in the report in the month of June, in particular: number of distributed medicines in the first report is  = 1111; when in the last report you indicated 1035 distributed medicines. Please indicate the reason for this difference?

3.      JUNE – the report Does not specifically indicate how many bottles of Sovaldi have been issued in the month of June? 1000 bottles? Please explain the Distributed number - 35/111 Bottle issue?

4.      JUNE – regarding the indicated 7 patients who stopped treatment, how many bottles of the medicine were distributed? Did the return of the medication happen?

5.      JULY-AUGUST – Please explain the status? Were there any beneficiaries selected for treatmens in July and August?

6.      SEPTEMBER  - From indicated 1000 beneficiaries  – 7 beneficiaries who stopped treatment, you have indicated the sum of beneficiaries  in total 993, In September report number of beneficiaries you indicated is 994 ? Does this mean that one beneficiary was involved?

7.      According to the treatment regime, there should be 3 bottles of sovaldi distributed to 1 beneficiary. In this report number of distributed medicines equals to 2742 (2742/994)? Please explain the mentioned discrepancy.

8.      When are the remaining patients completing their treatment? Please also indicate the number of pacients?

This questions are raised because of the inaccurate data in the reports. In accordance with the obligations undertaken by the Contract of Donation between the Ministry of Labour, Health and Social Affairs of Georgia and Ministry of Health of the Republic of Armenia, Please send us the detailed information with regard to the following questions:

 

1.      In total how many beneficiaries were involved in the treatment program?

2.      In total how many bottles of Sovaldi were distributed to the abovementioned beneficiaries?

3.      In total how many beneficiaries stopped the treatment? In total how many bottles were consumed by the mentioned beneficiaries? How many unused bottles were sent back?

4.      In total how many beneficiaries completed the treatment?

5.      In total how many beneficiaries achieved the positive result?

6.      In total how many bottles of sovaldi were left in stock? If so, were the expired bottles of sovaldi destroyed?

Please send the requested information as soon as possible (no later than November 10th,2017)
