Dear Sir/Madam,

With this letter I would like to kindly address you from the Ministry of Labour, Health and Social Affairs of Georgia to clarify several issues regarding US visa.

Please, be informed that Ms. Ani Sergeenko (passport number 11BB34336) and Ms. Nino Sergeenko (passport number: 11BB34740), daughters of the Minister of Labour, Health and Social Affairs of Georgia are invited by Sentara Norfolk General Hospital in Norfolk, Virginia (USA) during May 30 to June 24, 2017 to attend clinical rounds and participate in patient care discussions (please, see below the invitation letters and copies of passports).

Would you be kind to advise us which type of DS-160 nonimmigrant visa is to be filled? Do they need to provide other documentation in addition to 1.Passport; 2.DS-160 confirmation page; 3.visa application fee payment receipt; and 4.invitation letter? And the last, can we provide all requested documentation at the US consulate in Tbilisi instead of applicants?
