Dear Sir/Madam,
First of all, sorry for addressing you this way, because I do not know
whom
to address.
I want once again to extend our gratitude and sorry for delaying you
the
response.
We as the governmental institution are required by law to get approval
of
the Government for taking grants and as you may very well know it
takes
some time. But everything is already sorted and we received the
approval.
Regarding the Agreement we only need to have a minor change in the
first
sentence (indicated in red):
"On behalf of the Gilead Sciences, Inc. (“Gilead”), I am pleased to
inform Georgia,
represented by the Ministry of Labour, Health and Social Affairs of
Georgia (“Grantee”) that your request for a grant to support the
establishment of the Zugdidi Screening and HCV Management Centre
(“Project”), as set forth in greater detail in your Grant Request
Proposal
submitted on July 3, 2016, has been approved in the amount of USD
$641,922
(“Grant Funds”), payable as set forth in this letter agreement below
(“Letter Agreement”). Grant Funds must be used solely to support the
Project".
This change is caused by the law in force.
Also attached please find the bank requisites of MoH for the money
transfer.
Regarding the California Withholding forms, I fully understand that
you
can
not give us a tax advice, but we did not think that either Form 587
nor
590 was the appropriate form for us, as the foreign government, but
according to the information on www.ftb.ca.gov the most relevant form
is
588, could you please advice on this matter and indicate which form
should
we fill and send to you?
